And now to throw out a theory on Dream Estrana that I haven't heard mentioned, after some facts. I highly doubt that it will stop the argument between Impossible and DSOK, but...
It's clear that at least some of the people Rhue has killed have wound up there (even if, for whatever reason, you don't believe Rhue killed Midian, Jed is proof enough. The same goes for those who don't take Lexus's presence in the map of The City seriously. Also, I say some because there isn't any concrete evidence that everybody Rhue has ever killed goes there (I would expect to see a lot more Shadow Lords and Blood Lyn if that were the case)). However, it seems unlikely that absolutely everyone there was killed by Rhue: most of The City's population seems to consist of Estrana residents; in fact, it really looks like almost all of Estrana's population has wound up there (again, just my thoughts. We have no real way of knowing how many people were living in Estrana or of how many people are now living in The City). One of the only really logical explanation for their arrival in The City would be the manner in which, it's clear, a lot of Estrana's population perished: by the claws/fangs/what have you of the demons that were summoned by Sacrifa.
Now, if people that were killed indirectly by the Lord of the Pits wound up in a place that much resembled Estrana, one would have to ask why, since one would imagine that they would go to the Reaches, which, if Scatha is to be believed, definitely exist and are The Way's version of hell, in most people's beliefs. Perhaps the Lord of the Pits has created something of a foothold in Estrana's ruins and so the souls of the departed now wind up there. But how does this explain Jed's presence (possibly along with Midian's and Lexus's) in The City? Well, if you accept that Rhue's blade is a Shadow Sword, then one of the next logical steps is to link the Shadow Swords to the Lord of the Pits (Kloe speaks the common belief that the Shadow Swords are the embodiments of evil, and Kalmar
does say that he sensed the 'evil' of Traziun's Night Reaper). Perhaps killing someone with a Shadow Sword is the same as a demon's killing of someone: an indirect murder by the Lord of the Pits.
I get the feeling that holes could be pretty easy to punch in this theory, but there is some link between wanderers killed by Rhue's blade and the wanderers killed in Estrana's destruction.
Although I guess it could be possible that The City is just a wierd little place, mostly borne of Rhue's memories, both those he normally has and those he gained, but can't consciously access (Jed's definitiely and Midian's almost definitely). That one probably has even more holes that could be punched in it, though.
