I'm bored.... so here goes something of a quickly pasted together theory. And please remember, I thought of most of this crap before reading the topic, so just forgive anything that is a repeated theory. I've had to deal with reading things I stated literally over two years ago being reposted as original theories for quite a while, so I'm sure you can deal with it.
Rhue's Existence, Origin of the Shadow Sword
"Rhue" is Kura, simple enough.
Kura obtained a Shadow Sword (Phantom Slasher).
Proof
-Kavax (Kava) addressing Rhue as Kura
-The Red-Haired Woman speaks of a man who has possessed a sword for hundreds of years and tells us that a wielder of a Shadow Sword would not age.
She mentioned that the man searched for a way to save himself from being destroyed when the Sword absorbed too many auras and succeeded 'to a degree'.

Kura succeeded in finding a way to save his original aura from destruction upon the separation form his body and soul.
He learned how to survive as the Phantom Slasher avatar (this would be the shadowy guy) while the body was controlled by the massive amount of auras that the sword absorbed earlier.
The body and sword continued to gather more wanderer's auras and every once in a while a dominant aura would surface to control the body.
Proof
-WOMAN: I don't know.\.\. But I've heard that
Shadow Swords often "prefer" certain victims
to others\..\..\..\.\. Sometimes they like to be
used to kill,\.\. but other times they don't.

She is most likely saying that he has Kura's body but she knows the truth of how he saved himself and that it is no longer his aura within it.

These certain victims are the dominant auras. Jeruh possessed one of these auras and that is why he gained control of Kura's body.
The dominant aura could quite possibly always be the aura that is stashed in the Phantom's Edge's cell, which explains why Jeruh is in there.
The dominant aura is not fully aware of their own condition so they continue without knowledge of their own death.
Kavax (Kava) is present within The City because he died while in Kura's presence, whether or not he was killed by him, I have no idea.
Reminder
Keep in mind that the Woman did mention that the wielder of the Shadow Sword does not age physically, meaning that if child Jeruh had obtained the Phantom Slasher at Landorin, he would've stayed a child. Of course it could be true that the host will age until full maturity then will not decay due to age.
The original confrontation been Kura, Kava and Luma could have taken place on what was originally remembered as Landorin Stretch. This would explain how they returned to this place, the body and sword have been traveling The Way for a couple hundred years (the play says something about a number of generations I believe) so they could have circled around the entire path.
Midian, The Purple-Haired Enigma
Perhaps Midian is another dominant aura within Rhue, at least the one that came before Jeruh.
The easiest scenario here is that Kura (Controlled by Unknown Aura) came upon Midian one day and the sword demanded that the user (avatar or body itself) kill him for his aura.
Midian's aura was absorbed and took control of Kura's body.
Midian/Kura continued his quest for vengeance against Jeruh and came upon him one day.
Jeruh was killed for the shared reasons of him containing a dominant aura and Midian's vengeance.
Proof of Midian's Control of Kura

The headhunter actually still prove a point here. They don't sense auras, they merely know Kura when he was controlled by Midian, if you were looking for a certain person in such a great amount of space, wouldn't you ask other people if they've heard of/seen them?
HUNTER: What's wrong?\.\. I don't
ever remember you being so hostile.\|

HUNTER: What was their name
again?\.\. Jeruh wasn't it?
HUNTER: See ya,\.\. Midian.
I think he makes Midian's presence in Kura's body rather obvious and it follows the story fine.

The first headhunter questions Rhue, Dana and Wes about knowing Jeruh. They all say no and he begins to let them pass. He then turns and attacks. This would most likely be because Rhue may fit a brief description of the original Jeruh and because Wes says Rhue's name and the head hunter could possibly know that Jeruh was using that name.
The Landorin Massacre
-Scenario 1-
Jeruh loved Serena, this is obvious.
Serena disliked Jeruh, Lyrra tells us this.
All of the kids in the fist go off to play one night and leave Jeruh behind, told this as well.
Jeruh is starts looking for the other children because he feels lonely and runs across a dead body with a sword in it and a person who is just staring into space.
Since the Phantom Slasher sword would still be in Kura's possession, this would not be that sword, infact it could easily be a normal sword.
The person staring off into space could simply be in awe of the fact that he actually killed a man.
Jeruh takes the sword because he thinks he can show it off to the other kids to make some friends.
He then comes upon the other children making fun of him, he becomes filled with rage and attacks, none of the children have weapons so they are defenseless against an unskilled weakling of a swordsman like Jeruh.
Midian is thrown into the river and Jeruh runs off scared, probably incapable of telling whether or not what happened was real. He keeps running and eventually wakes up with the delusion that something else happened.
-Scenario 2-
Another possibility is that the sword at Landorin was one of the other swords, most likely Night Reaper. If it was indeed Night Reaper, Jeruh admitted to tossing the sword away when he began using the name Rhue. From here, Foreman Baller could've come across the sword and stored it in Lide.
-Scenario 3-
The shadow figure that was staring off into space was actually Kura's body and the dead man possessed a dominant aura, while this aura was in the process of taking over the body, Kura remained comatose (When Jeruh is killed, Kura/Rhue does seem to become dizzy or at least uneasy). Jeruh took the sword, murdered the children as we were told and ran off with the sword. We know that the sword won't automatically gain control of a body because Rhue has had his sword taken away before, albeit for short times. We do not know how long it was until the sword was abandoned by Jeruh but it was most likely not a very long time. The body and avatar, being drawn to the sword, retrieved it and continued along The Way.
